SIGN UP NOW AND CONSUMER CERTIFICATION
This form is used to acknowledge the agreement between your company and Pre-Employment Solutions, Inc. particularly in relation to the FCRA and the form provides the necessary information to establish your account.

Consent Form
This form is executed by the applicant to authorize Pre-Employment Solutions, Inc. , as the company's agent, to obtain all information pertaining to the applicant's background. 		 


Background Investigation Request Form
This form is used when requesting background investigations by fax from Pre-Employment Solutions, Inc.

Pre Adverse Action Letter/ Summary of Rights
This letter is sent to the applicant who fails to meet your qualifications due to information obtained from the background investigation and includes a summary of the applicant's rights as required by law (called the "Fair Credit Reporting Act"). Additionally, this letter solicits a response from the applicant as to the content of the report, enclosed with the letter.

Post Adverse Action Letter
This letter advises the applicant that employment has been denied wholly or partly due to the information obtained from the background investigation and discloses the Consumer Reporting Agency contact information along with other information required by the Fair Credit Reporting Act.
